I am currently accepting new clients who have coverage under the CHP Group. Prior to your appointment I will need your group plan number, date of birth and full name as shown on your health plan card. I will need to call to verify your coverage and co-pay information before your appointment. The CHP Group is only open Monday through Friday with occasional weekday closures. It is best to call at least 72 hours in advance of your desired appointment date with your health care provider information ready so I may verify coverage. Please note I am closed on Monday and Tuesday so all phone messages received during those days will be returned on Wednesday.
Your co-payment is due at time of service. I accept either cash or local check, but not credit or debit cards. I am happy to provide an email receipt for your records. CHP Group only pays for a 60 minute session, but if you prefer a longer, 90 minute session, I do extend sessions for an additional fee of $30 on top of your co-payment.
Please review my "Directions to Studio" page prior to your appointment for details on my location, as the door to the building is tucked between two businesses and is easy to miss.

*Please note: If you fail to give the 36 hours cancelation notice required prior to missing an appointment, you will be charged the full amount (co-pay + insurance covered portion), for your session. CHP Group does not pay for missed sessions due to client oversight.
